You may call our office at 615 340-4640 extension 0 or 224 to schedule a new patient consultation. We will need the patient’s full name, address, phone number, their employer information, social security number and date of birth. You will also need to provide us with their insurance information and diagnosis. Once we have scheduled the appointment, we request you fax ( 615 340-4642) us all medical information, including recent office notes, copies of ultra sounds, CTs, Pet scans, x-rays, lab work, and biopsies, etc. along with a copy of their insurance cards. We strive to get each patient in as quickly as possible and provide them with exceptional patient care.
